Looking for some help…I’ve been treating a 42 year-old male for several things since 8/28/17 with minimal improvement. He has had eight surgeries on his left knee and hip after a fall at work in 2010. His left knee has been totally replaced. There’s stiffness, swelling, and some lack of sensation to touch. He also had extreme difficulty falling asleep for 20+ years, has hearing loss in his left ear, and is completely infertile. He’s been on CHGZGJT off and on as needed, TXS + GCFZT + LGZGT, TXS + SQW, and most recently, YBJZT + FZ. Now, his sleep is only bad every 3rd or 4th night, which is the best it’s been in a long time, but his knee is still stiff, and he’s been getting headaches in his right eye. At his last visit, after taking YBJZT + FZ for a few days, he mentioned that he had a lot of stiffness on the left side of his neck and occiput, he was quite thirsty, and the formula made him ever-so-slightly nauseous about an hour after he took it. His most recent pulses, before and after YBJZT + FZ were: L1 slightly wiry tight, L2 thin wiry faint, L3 deep faint, R1 big hollow, R2 and R3 deep faint, both sides slightly rapid and slightly rolling up. What do you guys think?

Just briefly since I’m about to fall asleep, since you tried to treat the swelling from a MH angle, you could now try from a HQ angle, as in FJFLT.

Aaron Alan Chesterton, Hamish Brown, Peter Slipper, or anyone with an idea that helps me move on with this patient: do you have an idea regarding this case? What is most pronounced: falling asleep takes hours, wheezing of the lungs (dry sound of a heavy smoker), and a clenching feeling in the heart when exercising. What formula comes to mind with such superficial wiry pulses (see above post) which don’t drop on harmonising formulas? Harmonize even longer? Treat it as deficiency taxation, regarding the slight hollow quality? Or even go DHZCW? He is making really good progress, but I feel it could be better and other layers should be addressed, too, before he decides to quit his treatment. Thanks so much, collective brain!

I did put him back on CHGZGJT after his response to the YBJZT. I think I mentioned it to you before, but he tends to get relief that lasts 5-7 days and then the effects seem to wear off, which is just one more reason that I went back to a shaoyang formula yet again. This relief appears to mostly be coming from the acupuncture and bodywork that I do. He’s definitely not nauseous enough that I’d list it as a major deciding factor in choosing a formula; he only felt very very minor upset St about an hour after taking the YBJT + FZ, and other than that has yet to complain of any digestive stuff. Considering formulas related to YBJZT, would WTT be appropriate at all here?

I felt like we were getting the most done, although it hit a plateau, with TXS. However, that formula isn’t really in the pulses anymore, and the big hollow R1 is throwing me off.

It sounds like that effect is due to the acupuncture. Maybe try a WTJNT+HQi, FL

I’d try SQW + TXS. There are probably smarter options, but that one makes sense to me.

GZJNT with WT instead of FZ, and yes I hold the licence for that name, you will shortly receive an invoice for using it 😛

WT will give you more direct warming of the bones than FZ, but it seems a bit less rooting of floating yang, so if you want to focus it primarily on warming the joints you can just do a straight swap of FZ for WT, but if you want rooting of yang to be a prominent part of the treatment too you can always use them both, in my new patented formula Wu Fu Jiu Ni Tang
